If you have been a member of CHFM in the past and would like to participate again, please complete the Returning Member Form.

After receiving your form, we will add you to our mailing list and send you registration information. If you have been away from the ministry for a year or more, we will schedule an interview time to update you on ministry policies and procedures.

The deadline to submit your form for the Spring 2012 semester is Wednesday, December 14, 2011.
