Perhaps you have browsed our website or visited with a current member. If you think you might be interested in membership for the upcoming semester, please complete and submit the Prospective Member Form.

The deadline to submit your form for the Spring 2012 semester is Tuesday, December 13, 2011.


After receiving your form, we will contact you and schedule an interview to explain the ministry of CHFM in more detail.

Final interviews for the Spring semester will be Wednesday, December 14, 2011.
